To become a patient of the St Croix Regional Family Health Center, you first need to fill out our Release of Information form. This will allow us to gather your records from your previous facility. Once we receive your records one of our providers will review them to make sure we can provide you with the necessary care. Once we have these completed you will be contacted for scheduling.
